What the role is
What you will be working on
In this role, you will be involved in the policymaking, planning and implementation of home-based care services.
This will involve:
• Reviewing, developing and implementing policies and services to transform home-based care, so as to allow our growing population of seniors to receive seamless holistic care and age-in-place in the community, and avoid institutionalisation. This includes projecting of future demand and capacity required, developing new care models for greater quality of care and efficiency of delivery, and putting in place enablers (e.g. IT, data-sharing, appropriate financing approach).
• Ensuring programmes are run according to the policy objectives and guidelines, e.g. by reviewing operators’ performance and facilitating follow-up action where necessary.
• Monitoring and analysing population trends and citizen feedback, and conceptualising, developing and evaluating pilots to ensure services continue to meet evolving needs.
