To support the Board’s goals, the Regional Health Systems and Community Outreach Division collaborates closely with the community and healthcare partners to co-create regional and community-owned solutions to ensure good coverage and sustainable delivery of priority health promotion programmes in the community setting.
Key programme areas include active ageing, physical activity, nutrition, substance abuse and mental health. The initiatives cover the general population segments from children to seniors, as well as targeted outreach initiatives to ethnic minority groups and disadvantaged groups (such as those from lower socioeconomic status).
This person will partner Regional Health System (RHS) and community stakeholders in identifying the opportunities to implement and support initiatives for a healthier living environment. He/she will also build partnerships and link up partners into healthy ecosystems so that partners are able to “plug and play” the preventive health bundle programmes in their communities.
SCOPE OF WORK WILL INCLUDE BUT NOT LIMITED TO:
Partner the healthcare cluster and community partners on-ground teams to align delivery of programmes and services with national standards in support of Healthier SG
Collaborate with key partners from Regional Health Systems, government and non-governmental organisations, community partners, volunteers and vendors, to increase outreach to the population segments and strengthen quality of programmes/services offered in the community setting. Develop a systematic approach to partner outreach and relationship management and be able to maintain strong relationships with them to grow the partnership level.
Liaise with healthcare cluster and community partners to understand their programmes, data collection for the analytics & dashboarding, customer journey mapping and integration to HPB’s Preventive Health strategy.
Manage and monitor closely day-to-day operations of health promotion programmes delivered by outsourced vendors to ensure smooth implementation with the healthcare clusters in the community.
Prepare and update progresses of various projects collaborated with healthcare clusters and other key stakeholders to the Division, Senior Management, internal and external stakeholders.
REQUIREMENTS AND QUALIFICATIONS
Previous working experience as an Outreach and Partnerships Manager for at least 2 years;
A very good understanding of partnership development and relationship management. Ability to network easily and build strong collaborations with stakeholders;
Excellent communication and interpersonal skills and versatility in managing different levels of partnerships;
Strong organisational and project management skills; good multitasking skills but with attention to details;
Ability to work independently, and with teams;
Prior experience and partnerships with healthcare clusters will be an advantage
Passion to serve, particularly in the community setting
