You will join the Population Health Division (PHD) which is responsible for developing concepts, policies and working models with partners to encourage strong preventive health support in the community.

What you will be working on
You will:
• Develop strategies and plans for population health, with a specific focus on doing so through social and health community partnerships
• Formulate, review, and implement strategies and plans to grow the role of Regional Health Clusters in working with partners to improving population health outcomes
• Identify specific priorities and resident service journeys to improve population health outcomes and collaborate with partners to implement these efforts
