You will join the Population Health Division (PHD) which is responsible for developing strategy, concepts and plans to improve long-term population health.

What the role is
What you will be working on
You will guide a team to:
• Formulate, review, and implement strategies and plans based on evidence and research to identify target population segments, and improve the health of these segments
• Review and improve selected resident service journeys for key population segments, with partners, in order to improve uptake of preventive health, e.g. how to improve the Healthier SG enrolment and health plan journey
• Guide the development of resident-facing IT to meet population health goals, e.g. looking into how IT supports behavioural change and how this can be scaled.
• Organise and guide approach to evaluating these population health efforts, and how we take action on the findings
• Collaborate with partners to implement and monitor these efforts
