We want to empower residents in Singapore to attain optimal health, increase their quality and years of healthy life, and prevent illnesses, disability and premature death. To achieve this, HPB drives national health promotion and disease prevention programmes, and creates a supportive environment in Singapore where healthy lifestyle options are available and accessible to residents every day.
ABOUT THE DIVISION:
To support the Board’s goals, the Integrated Outreach East Division collaborates closely with the community and healthcare partners to co-create regional and community-owned solutions to ensure good coverage and sustainable delivery of priority health promotion programmes in the community setting. Key programme areas include active ageing, physical activity, nutrition, substance abuse and mental health. The initiatives cover the general population segments from children to seniors, as well as targeted outreach initiatives to ethnic minority groups and disadvantaged groups (such as those from lower socioeconomic status).
SCOPE OF WORK WILL INCLUDE BUT NOT LIMITED TO:
Collaborate with key partners from government and non-governmental organisations, Regional Health Systems, community partners, volunteers and vendors, to increase outreach to the population segments and strengthen quality of programmes/services offered in the community setting. Develop a systematic approach to partner outreach and relationship management, and be able to maintain strong relationships with them so as to grow the partnership level.
Plan, manage and implement HPB volunteer programme: Health Ambassador Network (HAN). Collaborate with Health Ambassadors (HA) who are volunteers with HPB to promote groundup health promotion efforts to residents. HAN also develop and engage the HA so that they can play a meaningful role supporting HPB vision.
Forecast, measure, and report the results of various projects to the Division, Senior Management, internal and external stakeholders.
