By 2030, almost a million Singaporeans will be aged 65 and above, and there will be an increased demand for aged care services (e.g. Senior Care Centres, Active Ageing Centres, Home Personal Care, Nursing Homes). However, the care landscape is fragmented today and care journeys for seniors can be uncoordinated with multiple providers delivering multiple assessments and care plans. The Sector Strategy and Development (SSD) team under the Ageing Planning Office is at the forefront of re-imagining the senior’s care journey, ensuring that seniors can receive seamless and coordinated care, and that service providers are uplifted to play such a role.
We will be introducing a new service model called the Integrated Community Care Provider (ICCP), where ICCPs would assess and plan for the senior’s care holistically, and be the designated point of contact to pull in and coordinate the aged care services that the senior needs. You will be at the forefront of re-imagining the senior’s care journey, as well as developing and implementing new service models. In addition, your work will also involve the building of capabilities – such as data-sharing, digitalisation, productivity and innovation – required to deliver the ICCP service model and other shifts in the long-term care landscape, to meet future care models.

What you will be working on
In this role, you will be involved in the policymaking, planning and implementation of the aged care sector landscape review. This will require:
- strategic and blue-sky thinking to reimagine the care models and support for seniors,
- a good grounding of a senior’s journey in receiving care across acute, primary, and long-term care settings,
- an ability to work through the operational details needed to implement these strategies (e.g. budgeting and finance, IT/data, planning/analysis based on projections, regulatory and service requirements and transitions), and
- establishing enablers and structures within the long-term care sector and/or with the larger healthcare sector that will facilitate smooth policy implementation to bring about better outcomes for the seniors.
